My Medicines Forms Available for Online Completion

CredibleMeds is pleased to announce the availability of the My Medicines Forms service again and apologize to anyone inconvenienced while the tool was being updated.  We encourage everyone to carry a complete and up-to-date list of the medicines they are taking in their wallet/purse in case of an emergency.  When completing the My Medicines Forms online, CredibleMeds will automatically check to see if any of your medicines are on one of the QTdrugs lists of drugs that have a risk of causing heart rhythm problems.  Also, if one of the medicines on your list is subsequently added to one of our QTdrugs lists, you will be immediately notified.
